Magical girls injured in a kaiju disaster are carried into a makeshift emergency hospital every day. You decide who to admit and who to turn away.
Magical Girl ER, a solo-developed horror medical sim, launched on Steam on October 26, 2026. The full release comes after a demo that had been available since July. It is single-player, with English on-screen text and subtitles. As of the day after launch, 89% of its 46 Steam user reviews were positive.
You run the hospital for one week as its acting director. You examine and treat the patients you admit, but beds are limited.

Exams use tools like a thermometer and a stethoscope. Patients may be hiding symptoms such as physical trauma, internal damage, poisoning, mental instability, or magical contamination. Your clues come from exam results, conversations, the patient’s appearance, and paperwork. You piece them together to work out what is happening to the patient’s body.

A hidden symptom can only be confirmed by examining a patient you have already admitted. So you decide who gets one of the limited beds before that, which is likely to make every day’s admissions tense.
You manage two things: how much people trust the hospital, and whether patients live or die. Your choices lead to one of 11 endings. There are also events such as newspaper articles, encounters with visitors, and horror scenes.
The game shows injured magical girls, blood, and death. Some scenes force difficult medical decisions, and there are sudden visual changes and loud sounds. According to the developer’s content notice, there is no excessively graphic content such as dismemberment or exposed organs, and no sexual content.
Developer noranuco said in a post on X that the game was developed solo.
